ADMISSION CRITERIA
Prospective residents who are able and/or their families will be encouraged to visit Hill House. They will be given a tour and have an
opportunity to ask any questions they may have.
The individual must be:
- an adult (age 18 or over);
- a resident who lives within our Central LHINS (Local Health Integration Network) or has family that lives within our LHINS;
- diagnosed with a terminal illness; and
- deemed to have a life expectancy of less than one month.
Referrals will be accepted from attending physicians, hospitals, homes for the aged, community hospice organizations, other health care professionals or family members.
